FUT vs. FUE hair transplant: What are the differences? A summary of the advantages, limitations, and how to choose the right one for you.

Permanent hair transplantation to address hair loss, thinning hair, and baldness due to genetics currently utilizes two main internationally recognized techniques: FUE (Follicular Unit Extraction) and FUT (Follicular Unit Transplantation). Although both methods aim to transfer healthy hair follicle cells from the donor area to the problematic area, the harvesting process, recovery, and scarring differ significantly.
FUE (Follicular Unit Extraction) technique
FUE is a hair transplant technique that involves using a special small drill (micro-punch) with a diameter of approximately 0.7–0.9 millimeters to drill deep around the hair follicles in the occipital area to precisely extract hair grafts one by one.
Advantages of FUE
There are no long, linear scars; the resulting wounds are just small spots scattered across the nape of the neck, which will scab over and heal on their own within 5–7 days.
Recovery is faster and less painful. There is no major biopsy, so there is no skin pulling or stretching. You can return to your daily life quickly.
Freedom to choose your hairstyle; you can get a short haircut, skinhead, or fade/undercut with virtually no visible scarring.
Limitations of FUE.
It takes quite a long time to do because each strand of hair has to be collected one by one.
In the case of standard large-volume hair transplantation (Shaven FUE), it may be necessary to shave the hair at the back of the head short to ensure precise follicle penetration (unless the Non-Shaven or Long-Hair FUE technique is chosen).
It requires a very high level of expertise from the doctor to control the direction and depth of the hair follicle injection to prevent damage to the hair roots (low transfer rate).
FUT (Follicular Unit Transplantation) technique
FUT, also known as the "Strip Method," is a traditional technique where a doctor surgically removes a long strip of scalp from the nape of the neck. A team of medical assistants then examines this strip of scalp to separate the hair follicle cells into individual clumps under a stereomicroscope, while the doctor sutures the incision at the nape of the neck closed.
Advantages of FUT
This method allows for a large volume of grafts to be administered in a single session , making it suitable for those with extensive bald areas requiring up to 3,000–4,000 grafts or more.
The high survival rate of hair follicles means that careful microscopic examination reduces the risk of damage to the tissue surrounding the hair follicle.
There's no need to shave a large area of the nape of the neck; only a thin strip of scalp is cut. The hair from above can immediately fall down and cover the stitches after the procedure.
Limitations of FUT
Even with advanced suture techniques (Trichophytic Closure) to allow hair to grow through the scar, a visible surgical line will remain if the hair is cut very short.
The recovery period is longer, with tightness and pain around the surgical site for the first 1-2 weeks. Sutures need to be removed approximately 10-14 days after surgery.
Limitations in scalp elasticity: If the scalp at the nape of the neck is too taut, it will not be possible to cut the skin strip.
Table comparing the differences between FUE and FUT.
Points of comparison | FUE (Follicular Unit Extraction) | FUT (Follicular Unit Transplantation) |
How to preserve hair roots. | Harvest each clump individually using a 0.7–0.9 mm drill bit. | A surgical procedure involves removing a long strip of scalp from the occipital bone. |
Types of scars | Small, scattered, circular sores that heal quickly. | A long, linear scar at the nape of the neck. |
Suturing and removing sutures. | No stitches needed. No suture removal required. | The wound needs to be sutured closed, and the stitches will be removed 10–14 days after the procedure. |
Level of pain and tension at the wound site. | Most people recover within 3–5 days. | The surgical site is more stiff and sore. |
Appropriateness of the hairstyle | It works with any hairstyle, including short, shaved-side hair. | You should keep your hair at least 1–2 inches long to cover the scar. |
Preparing the hair at the nape of the neck. | Nowadays, you have the option of either shaving short or going long-haired. | Shave only along the line where the skin will be cut. |
Is FUE or FUT better?
In medicine, there is no single best method; rather, it depends on which approach best suits the anatomical needs and goals of each individual. These include:
FUE is suitable for those who want small incisions, quick recovery, minimal downtime, prefer short hair, or those with high scalp tension at the nape of the neck that prevents tissue removal.
FUT is suitable for patients with extensive baldness who require a large number of hair grafts at once, especially those who already have long hair and are not concerned about long scars.
Currently, the FUE technique is becoming increasingly popular due to its high precision, very small scars, and advanced techniques like Long-Hair FUE that don't require cutting or shaving the hair, allowing patients to return to their normal lives immediately without being noticed.
5 Important Criteria for Choosing a Hair Transplant Method Before Making a Decision
When evaluating and selecting the correct hair transplant method, several factors should be considered, including the following:
The severity of the problem and the area to be transplanted depend on the specific needs. For hairline or crown areas, FUE (Foam Extraction) is often sufficient and provides high precision. However, if the bald area is very large, measuring 5–7 on the Norwood Scale, careful consideration of the occipital area may be necessary.
Scalp density and elasticity at the occipital lobe (Donor Area Evaluation): The doctor needs to measure the hair follicle density (Graft Density). If the occipital lobe is thin, extracting too much FUE may make the occipital lobe appear balding. However, if the scalp is too tight, FUT cannot be performed either.
Hair styling habits and personal preferences: If you like short haircuts, side fades, or plan to cut your hair short in the future, the FUE technique is a safe solution, avoiding the problem of bothersome scarring.
For those who need to return to work quickly, have outdoor activities, or want to avoid being noticed by others, FUE or Long-Hair FUE is a more suitable option for a smoother recovery.
Long-term treatment plans: Hereditary hair loss may continue in the future. Careful hair graft management (Donor Management) is therefore crucial to ensure that hair resources are available for future use.
The success of hair transplantation doesn't depend solely on the name of the technique, but rather on in-depth analysis and customized hairline design to ensure the new hairline naturally complements the facial structure, gender, and age.
